Clarity Community Connections

Clarity Community Connections (CCC), a nonprofit organization in Londonderry, NH, that provides case management services to individuals who are in need of resources, information, and support but do not meet criteria for case management through government agencies. CCC serves as a safety net for those who would otherwise slip through the cracks, helping them to effectively connect with their community and receive the services they need

Clarity Community Connections currently offers case management services to those in need as well as four different programs. Our Mommy & Me program is for mothers who do not have custody of their children and would benefit from a weekly visit in a safe, neutral, supervised environment. Our Good Friend program is for women who have recently spent time incarcerated or in substance use disorder programs and are trying to start over. We provide them with resources in their community to get back on their feet. Our Break Through program provides case management for young adults who are trying to navigate the path from high school or college graduation to adulthood. Our Community Cupboard is available to anyone that needs food assistance during these trying and uncertain times.
